HOW POCKET MED LIST HELPS

Keep medication lists, dose history, and care updates organized between visits.

About these estimates

Published U.S. estimates from earlier research—not current-year counts or outcomes measured in Pocket Med List users.

  1. Medication adherence: about 50%. CDC’s May 2024 guidance cites an estimate that about half of chronic disease medications are not taken as prescribed. Its underlying reference is a 2012 systematic review. This is a broad estimate, not a current-year population measurement. Read CDC’s guidance ↗
  2. Mortality: an estimated 125,000 deaths annually. The American Heart Association’s page, reviewed January 18, 2024, cites this U.S. estimate for poor medication adherence. It is a longstanding estimate, not a new annual death count. Read the AHA guidance ↗
  3. Healthcare costs: up to $300 billion annually. The same AHA guidance describes additional U.S. medical visits and hospital care associated with poor adherence. CDC’s 2017 report cites a broader $100–$300 billion range. These historical estimates are not current spending totals or projected app savings. Read the CDC cost context ↗

Our role. Pocket Med List supports organization, reminders, and information sharing. These statistics do not establish that the app prevents deaths or hospital visits, improves clinical outcomes, or reduces healthcare costs. Clinical and economic outcomes would need a separate evaluation.

What is the Pocket Med List app?

Pocket Med List is a medication-management app for iPhone and Android, built by an emergency-department pharmacist. Keep medication names, strengths, directions, reminders, dose history, health readings, and caregiver access organized on your phone. Download the official app using the App Store or Google Play buttons on this website.

Is Pocket Med List free?

Yes. Your medication list, reminders, dose logging, safety checks, care-team sharing, and weekly summary are free. Optional Pro adds features such as extended health history, monthly and custom reports, and PDF export. The app shows the current plans and local prices before you subscribe.

Can I use it on iPhone and Android?

Yes. Pocket Med List is available on the Apple App Store and Google Play. Use the download buttons on this page to open the correct store.

Can I manage medications for someone who does not have a phone?

Yes. Add a person you care for and keep their medication information separate on your phone. Your own profile plus one additional person are free; managing more people requires the applicable paid plan. You can also invite someone with their own account to your care team.

Can I record blood pressure and blood glucose?

Yes. Enter readings from your blood-pressure cuff or glucose meter and review them alongside your medication information. The app does not measure these readings itself. Extended history and PDF reports are optional Pro features.

Can I bring my list to a medical appointment?

Yes. Review your medication names, strengths, and directions before the visit, then show your list to your clinician. Optional Pro reports can be exported as a PDF for printing or sharing.

Does it replace advice from my pharmacist or doctor?

No. Keep your list consistent with your prescription labels and your clinician’s instructions. Safety checks are an extra prompt to review information, not a complete clinical assessment. Speak with your pharmacist or prescriber before changing treatment.

Who can see my medication information?

You choose whom to invite and what access to grant through care-team sharing, and you can revoke that access. Read our Privacy Policy for details on how the service handles information.

How can a clinic or health plan recommend it?

Visit our healthcare collaborations page to explore patient introductions, caregiver support, and a focused adoption rollout. Contact our team to discuss your population, workflow, and requirements.
